- Complete a DAS Grievance Form.
- ​Submit the form to the Director of Disability Access Services. You can hand deliver, mail, or email the form.

If you do not want to submit the grievance form, you can schedule a phone, virtual, or face-to-face interview instead. To schedule an interview, use the email address or phone number listed above.
Once the Director of DAS has received your grievance, the director will review it. Within 48 business hours of receiving the formal grievance, the director will contact you to schedule a time to discuss the grievance. The director may ask you for more information and will discuss potential resolutions to your grievance.
