Introduction: This is step one in the process for seeking approval for either an Independent Study or Directed Self-Study contract. It is assumed that you have already spoken to the student about the possibility of either an Independent Study or Directed Self-Study. Please fill out the appropriate boxes, including selecting if you are planning to do an independent study or directed self-study with a student. At the end of the form, you’ll select the department chair. Once you submit the form, it will go to the student for their review and acceptance and then go to the Department Chair.
